Location: Breckenridge, Minnesota

End Date: 09-14-2026

Contract Type: Contract

Shift Type: Rotating shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, or 10:00 PM - 6:30 AM with cycle scheduling, every other weekend and holiday

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate care to residents in a long-term care setting
Support residents with conditions such as CHF, COPD, Alzheimer's, Parkinson's, and Diabetes
Collaborate with the charge nurse and care team to ensure quality outcomes
Maintain accurate documentation using PointClickCare EMR
Foster a warm, home-like environment for all residents

Qualifications

Active LPN/LVN license in Minnesota
Current BLS certification
Prior long-term care experience preferred
Strong teamwork and communication skills

Knowledge & Skills

PointClickCare EMR proficiency
Understanding of common long-term care diagnoses
Ability to work independently within scope of practice
Compassionate bedside manner
