Contract Type: Travel Contract

Shift Type: Days (Monday-Friday), no weekends or holidays required

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct home health physical therapy visits, including start-of-care evaluations for orthopedic patients
Develop and implement individualized treatment plans for geriatric patients with comorbidities such as CHF and COPD
Document patient progress and care using HCHB (HomeCare HomeBase) EMR
Coordinate with nursing and therapy team members to ensure comprehensive patient care
Travel within Tehama County (approx. 40 miles per day) to provide services in patients' homes

Qualifications

2+ years of experience preferred, but will consider less with home health experience
BLS certification required
Active California Physical Therapist license
Home Health experience strongly preferred

Knowledge & Skills

Proficiency with HCHB (HomeCare HomeBase) EMR
Experience with orthopedic rehabilitation and geriatric patient care
Strong clinical assessment and documentation skills
Ability to work independently and manage a daily caseload of 5 visits
