Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide comprehensive care coordination for patients in need of mental health support
Conduct assessments and develop personalized care plans
Collaborate with interdisciplinary teams to ensure holistic patient care
Facilitate 5150 evaluations and coordinate 72-hour holds when necessary
Advocate for patients' rights and access to community resources
Document all interactions and interventions accurately and timely

Qualifications

Active California LCSW license required
Current BLS certification
Must hold current 5150 certification in California
Prior experience with Dignity Health or CommonSpirit facilities is required
Two supervisory references (one within 12 months, one within 3 years)
Not directly employed by any CommonSpirit facility within the past year

Education

Master's degree in Social Work

Knowledge & Skills

Expertise in care coordination and case management
Strong understanding of mental health crisis intervention
Excellent communication and documentation skills
Ability to work collaboratively with multidisciplinary teams
Proficiency in electronic health records
