Physical Therapist - Home Health
Location: Louisville, KY
Start Date: 06-29-2026
End Date: 09-28-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide skilled physical therapy services to patients in their homes
- Perform comprehensive evaluations, develop treatment plans, and implement interventions
- Conduct start of care visits, recertifications, and discharges as needed
- Educate patients and families on home exercise programs and safety
- Collaborate with a multidisciplinary team to ensure optimal patient outcomes
- Document patient progress accurately and timely using provided tablet
Qualifications
- Active Physical Therapist license in Kentucky (or compact eligibility)
- Current BLS certification
- Prefer at least 1 year of experience, but new grads may be considered
- Home health experience is a plus
- Valid driver's license and reliable transportation
Education
- Graduate from an accredited Physical Therapy program
Knowledge & Skills
- Expertise in evaluating and treating orthopedic, CHF, and pneumonia patients
- Ability to work independently in a home health setting
- Excellent communication and patient education skills
- Proficient with electronic documentation and telehealth tools
