Contract Type: Travel Contract

Shift Type: Monday-Friday, Day shift

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide comprehensive physical therapy assessments and treatments in patients' homes
Develop and implement individualized care plans to improve mobility and function
Educate patients and families on therapeutic exercises and home safety
Collaborate with interdisciplinary team including RNs and OTs
Document patient progress accurately in HCHB EMR
Perform start-of-care visits for orthopedic patients when nursing is not ordered

Qualifications

Active Physical Therapy license in California
BLS certification required
Minimum 2 years of home health experience (or less with home health experience)
Experience with HCHB EMR preferred
Ability to travel within Tehama County (approximately 40 miles per day)

Knowledge & Skills

Knowledge of orthopedic and geriatric patient care
Skills in home health therapy and patient education
Proficiency with HCHB and other EMR systems
Strong communication and documentation skills
Ability to work independently and manage a daily caseload of 5 visits
