Roles & Responsibilities

• Provide skilled physical therapy services to patients in their homes across a 60-mile radius around Valley City
• Develop and implement personalized treatment plans for patients recovering from surgeries, falls, and other conditions
• Collaborate with patients and families to promote independence and improve quality of life
• Document patient progress accurately using the HomeCare/HomeBase EMR system
• Educate patients on exercises and techniques to aid recovery and prevent future injury
• Travel to patient homes, ensuring safe and timely care delivery

Qualifications

• Valid ND Physical Therapy License (required)
• Current BLS certification (required)
• Minimum 1 year of experience as a Physical Therapist
• Previous home health experience strongly preferred
• Valid Driver's License and current Auto Insurance
• Willingness to travel up to 60 miles from Valley City for patient visits
• No previous travel experience required - we welcome first-time travelers!

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Proficiency in HomeCare/HomeBase EMR or similar systems
• Excellent communication and patient education abilities
• Ability to work independently and manage a caseload of 5-7 patients per day
• Comfortable driving to various patient locations in all weather conditions
