Location: Woodland, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturday shifts once a month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and implement individualized treatment plans based on physician orders
Instruct patients and families on therapeutic exercises and use of equipment
Monitor and document patient progress, adjusting treatments as needed
Collaborate with the healthcare team to ensure coordinated care
Educate caregivers on therapy goals and home exercise programs
Participate in case conferences and peer consultations
Ensure compliance with Medicare, Title 22, and other regulations

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute care, home health, or rehab setting
Active California PT license
Current BLS certification
Valid driver's license and reliable transportation for home visits
Excellent communication and organizational skills

Knowledge & Skills

Strong patient assessment and clinical reasoning skills
Knowledge of home health regulations and Medicare conditions of participation
Ability to educate and motivate patients and families
Proficiency in documenting patient care and progress
Team collaboration and case management skills
