Licensed Clinical Social Worker (LCSW) - Care Coordination
Location: Carmichael, California
Start Date: 07-07-2026

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (40 hours/week)
Hours per Week: 40
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Coordinate comprehensive care plans for patients in a psychiatric setting
- Conduct assessments and facilitate 72-hour holds as needed (5150 certified)
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
- Provide psychosocial support and crisis intervention
- Maintain accurate and timely documentation
Qualifications
- Active California LCSW license
- Current BLS certification
- 5150 Certification in California (or willingness to obtain prior to start)
- Prior experience with Dignity Health or CommonSpirit facilities preferred
- Minimum 2 supervisor references (charge accepted) within the last 12 months or 3 years
Knowledge & Skills
- Expertise in psychiatric social work and crisis intervention
- Strong care coordination and case management skills
- Excellent communication and interpersonal abilities
- Familiarity with CA Dignity Health systems and protocols
- Ability to work independently and as part of a team
