Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and home environment
Develop and implement individualized treatment plans with measurable goals
Guide patients and families through therapeutic exercises and gait training
Educate caregivers on proper use of therapeutic equipment and devices
Collaborate with physicians and interdisciplinary teams to optimize patient outcomes
Document progress and communicate changes in patient condition promptly

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Patient assessment and home environment evaluation
Therapeutic exercise programming and gait training
Range of motion and mobility assessment
Caregiver education and family support
Medicare and Medi-Cal compliance knowledge
Clear documentation and clinical reporting
