Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct Start of Care visits especially with orthopedic patients
Develop personalized treatment plans for geriatric patients with multiple conditions
Help patients recover from joint replacements and manage chronic illnesses
Document care using HomeCare HomeBase EMR system
Travel throughout Tehama County to deliver quality care where patients need it most

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience
Comfortable working independently in home settings

Knowledge & Skills

Physical Therapy expertise in home health settings
Proficiency with HomeCare HomeBase EMR system
Strong assessment and care planning skills
Excellent communication and patient education abilities
Comfortable with tablet-based documentation
