Roles & Responsibilities

• Provide expert home health physical therapy services, focusing on orthopedic rehab and geriatric care
• Perform Start-of-Care (SOC) visits and manage a daily caseload (approximately 5 visits per day)
• Document all patient encounters thoroughly in the Netsmart/Cerner EMR using the company-provided laptop
• Coordinate patient care with intake RN, manager, nurse aides, and other interdisciplinary team members
• Deliver safe, patient-centered interventions for joint replacements and patients with chronic conditions such as CHF and COPD
• Travel within the service area of Tehama County to bring quality rehab care directly to patients' homes

Qualifications

• Active Physical Therapy license (license submission is required)
• BLS certification (required)
• Minimum 2 years of relevant physical therapy experience (explicitly requested)
• Experience with home health therapy, including SOC visits and orthopedic patient care
• Comfortable documenting in Netsmart and/or Cerner EMR systems

Knowledge & Skills

• Orthopedic rehabilitation expertise (joint replacements and post-op care)
• Geriatric care knowledge including management of multiple co-morbidities
• Proficiency with Netsmart and Cerner electronic medical records
• Strong clinical documentation and case management skills
• Ability to manage a steady daily visit load and prioritize patient needs
