Roles & Responsibilities

• Provide high-quality occupational therapy care in patients' homes across an assigned geographical region
• Complete thorough OT evaluations and develop individualized treatment plans
• Accurately complete OASIS documentation and use Cerner for charting
• Collaborate with PT, SLP, OTA/PTA and rehab aides to optimize patient outcomes
• Manage caseloads efficiently while prioritizing patient safety and functional goals
• Participate in a 1.5-2 week orientation to learn unit-specific workflows and expectations
• Communicate clearly with patients, families, and interdisciplinary teams about progress and discharge planning
• Cover weekend rotation every third weekend and provide on-call support as required

Qualifications

• Licensed Occupational Therapist eligible to practice in California
• BLS (Basic Life Support) certification required
• Experience with OASIS documentation (required)
• Experience using Cerner or similar EMR for clinical documentation
• Minimum 1 year home-setting or PT/OT/SLP experience preferred; job listing notes 2 years experience preferred
• Comfort collaborating with rehab aides, OTA/PTA and interdisciplinary teams
• Ability to work within a geographic region and travel to patient homes reliably
• Professional appearance (business attire with white lab coat; no jeans)

Knowledge & Skills

• Strong knowledge of home health OASIS requirements and post-acute care documentation
• Assessment and treatment planning for ortho, joint, spinal injuries and stroke rehab
• Skilled in patient safety, mobility, ADL training and adaptive equipment recommendations
• Excellent clinical reasoning and time management for outpatient/home health caseloads
• Effective interdisciplinary communication and teamwork skills
• Familiarity with Cerner EMR workflows and electronic charting best practices
• Comfort orienting to a variety of home environments and adapting treatment accordingly
• Ability to teach, mentor and coordinate with rehab aides and support staff
