Roles & Responsibilities

• Deliver patient-centered physical therapy evaluation and skilled treatment in a skilled nursing facility
• Develop individualized rehab plans that help residents regain mobility, independence, and confidence
• Use evidence-based interventions like therapeutic exercise, gait training, balance and transfer training
• Collaborate with nursing, OT, SLP and other team members to coordinate safe, effective care
• Document progress clearly and timely in the facility’s medical record and meet regulatory requirements
• Provide family and caregiver education to promote continuity of care after discharge
• Participate in care planning meetings and contribute to quality improvement initiatives

Qualifications

• Licensed Physical Therapist in Ohio (or eligible for Ohio licensure)
• Doctor of Physical Therapy (DPT) preferred
• New graduates welcome — any level of experience accepted, including New Grad
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in a SNF setting and managing caseloads across varying acuity
• Acceptable background check and immunizations per facility policy

Education

• Doctor of Physical Therapy (DPT) or equivalent

Knowledge & Skills

• Skilled in assessment of mobility, balance, gait, transfers, and functional goals
• Proficiency in developing safe, measurable, and reimbursable treatment plans
• Strong documentation skills and familiarity with common EHR systems
• Ability to work collaboratively within interdisciplinary teams
• Knowledge of Medicare, MDS, and SNF regulatory documentation expectations
• Excellent communication and patient education skills
