Roles & Responsibilities

• Provide skilled occupational therapy interventions to residents in a Skilled Nursing Facility
• Assess patient function and develop individualized treatment plans that promote independence
• Document therapy progress and outcomes clearly and timely in the clinical records
• Collaborate with nurses, therapists, and interdisciplinary team members to coordinate care
• Educate patients and families on strategies, adaptive equipment, and home safety
• Support rehab assistants and contribute to a positive, patient-centered environment

Qualifications

• Certified Occupational Therapy Assistant (COTA) credential required
• State licensure, registration, or ability to obtain required state permit for Ohio
• Open to all experience levels — new grads encouraged to apply
• Current immunizations and ability to pass background and TB screening
• Strong communication skills and comfort working in a fast-paced SNF setting
• BLS/CPR certification preferred or willingness to obtain

Education

• Completion of an accredited Occupational Therapy Assistant (OTA) program
• Active COTA certification

Knowledge & Skills

• Familiarity with Skilled Nursing Facility (SNF) care and common geriatric diagnoses
• Skilled in ADL retraining, adaptive equipment recommendations, and mobility strategies
• Clear, concise clinical documentation and care plan development
• Ability to work collaboratively within an interdisciplinary team
• Comfort with clinical problem solving and adapting interventions to individual needs
