Roles & Responsibilities
- Provide compassionate clinical social work services to patients and families
- Conduct thorough psychosocial assessments and develop care plans
- Coordinate care with interdisciplinary teams to ensure comprehensive support
- Facilitate 5150 evaluations and involuntary holds when clinically appropriate
- Connect patients with vital community resources and support services
- Advocate for patients navigating complex healthcare and mental health systems
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities strongly preferred
- Strong clinical assessment and crisis intervention skills
Knowledge & Skills
- Expertise in involuntary psychiatric holds and 5150 processes
- Understanding of mental health crisis intervention
- Knowledge of community resources and support systems
- Strong documentation and care coordination abilities
- Compassionate approach to patient advocacy
