Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct OASIS assessments and start-of-care evaluations for new patients
Develop personalized treatment plans for elderly patients recovering from falls and fractures
Manage a manageable caseload of 4-6 patient visits daily using a point-based system
Document visits accurately with tablet technology for seamless care coordination

Qualifications

Licensed Physical Therapist in Indiana
Experience with OASIS assessments and start-of-care documentation
Comfortable working independently in rural community settings
Strong organizational skills to manage daily visits and documentation

Knowledge & Skills

Physical Therapy expertise with focus on geriatric care
Proficiency in OASIS assessments and start-of-care processes
Ability to build rapport with elderly patients and their families
Familiarity with tablet-based documentation systems
Adaptability to serve both rural areas and mid-sized towns
