Roles & Responsibilities
- Deliver personalized physical therapy services directly in patients' homes
- Develop individualized treatment plans that promote recovery and independence
- Collaborate with interdisciplinary care teams to ensure seamless patient outcomes
- Educate patients and families on exercises and techniques for continued progress
- Document care accurately to support quality and compliance standards
Qualifications
- Active California Physical Therapist license
- BLS certification
- Home health experience preferred
- Strong communication and time-management skills
