Roles & Responsibilities
- Provide compassionate physical therapy services directly in patients' homes across Yolo County
- Assess and evaluate each patient's therapeutic needs and create personalized rehabilitation plans
- Guide patients and families through exercises, mobility training, and use of therapeutic equipment
- Collaborate with physicians and care teams to track progress and adjust treatment goals
- Supervise home health aides and coordinate with other care providers for comprehensive support
- Document clinical progress and communicate changes in patient condition to the care team
Qualifications
- Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Valid driver's license and reliable transportation for home visits
Knowledge & Skills
- Physical Therapy assessment and treatment planning
- Home safety evaluation and patient mobility training
- Patient and family education on therapeutic equipment
- Documentation and clinical reporting
- Medicare and Medi-Cal compliance knowledge
