Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate care coordination for patients in need of mental health support
Complete thorough assessments and develop individualized care plans
Collaborate with interdisciplinary teams to ensure seamless patient care
Support patients and families through the 5150 evaluation process when needed
Document all clinical interactions accurately and timely

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities required

Knowledge & Skills

Strong assessment and crisis intervention skills
Knowledge of involuntary hold procedures and mental health regulations
Excellent communication and documentation abilities
Ability to work collaboratively within a care coordination team
