Roles & Responsibilities

Provide individualized physical therapy assessments and treatments to home health patients
Develop and implement customized care plans to improve mobility, strength, and independence
Educate patients and families on exercises and techniques for ongoing recovery
Collaborate with physicians, nurses, and other healthcare professionals for coordinated care
Document patient progress accurately and maintain compliance with regulatory standards
Travel within the Bakersfield area to deliver care in patients' homes

Qualifications

Active California Physical Therapy license
Current BLS certification
Minimum 1 year of home health experience preferred
Valid driver's license and reliable transportation
Compassionate and patient-centered approach to care

Knowledge & Skills

Strong clinical skills in physical therapy evaluation and treatment
Excellent communication and interpersonal abilities
Time management and ability to work independently
Understanding of home health regulations and documentation
Adaptability to various home environments and patient conditions
