End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekend shifts (8:00 AM - 5:00 PM)

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive physical therapy evaluations and develop personalized treatment plans
Perform start of care, recertifications, and discharge visits for home health patients
Manage a caseload of patients with diagnoses including CHF, orthopedic conditions, and pneumonia
Collaborate with a dedicated clinical manager and 24/7 educator support
Document patient progress and communicate with the care team using provided phone and tablet

Qualifications

Active Physical Therapist license in Kentucky or compact eligibility
BLS certification required
At least one year of home health experience preferred; new graduates are welcome
Must be comfortable with home health settings and driving to patient locations

Knowledge & Skills

Strong clinical assessment and problem-solving skills
Experience with OASIS documentation and home health regulations
Knowledge of Medicare guidelines and compliance
Excellent communication and time management abilities
Proficiency with electronic health records and mobile devices
