Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Develop individualized treatment plans to improve mobility and function
Educate patients and families on exercises and safety techniques
Document patient progress accurately and timely using HomeCare/HomeBase EMR
Collaborate with interdisciplinary team to ensure comprehensive care
Travel within a 60-mile radius to deliver care across Valley City and surrounding areas

Qualifications

Active North Dakota Physical Therapy License required
Current BLS certification (American Heart Association)
Valid Driver's License and Auto Insurance
Minimum 1 year of recent home health experience
Previous travel experience is not required but welcomed
Comfortable using HomeCare/HomeBase or similar EMR systems

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently and manage a caseload
Familiarity with home health regulations and documentation standards
Time management and organizational skills to handle 5-7 patients per day
