Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive Oasis Assessments and initiate Start of Care for home health patients
Develop and implement individualized physical therapy plans for elderly patients recovering from falls and fractures
Collaborate with interdisciplinary team to ensure coordinated care across four counties
Document patient progress and visit details using a tablet-based system

Qualifications

Valid Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care is required
Ability to travel between Noble, Lagrange, Steuben, and DeKalb counties
Comfortable with tablet documentation and point-based visit tracking

Knowledge & Skills

Expertise in Oasis Assessment and Start of Care procedures
Strong time management and organizational skills for managing 4-6 patient visits daily
Compassionate approach to treating elderly patients with fall-related injuries and fractures
Ability to work independently in a rural home health setting
