Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive OASIS assessments to develop personalized care plans
Perform Start of Care evaluations for new patients across four rural counties
Provide hands-on physical therapy to help patients recover from falls and fractures
Manage a manageable caseload of 4-6 patient visits daily using our point system
Document visits efficiently using tablet technology for seamless tracking

Qualifications

Current Physical Therapist license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working independently in rural community settings
Strong organizational skills to manage daily patient visits
