Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services directly in patients' homes across a 60-mile radius
Help patients recover from surgeries, falls, and other conditions with compassionate, one-on-one care
Collaborate with Home Health and Hospice teams to support holistic patient recovery
Document care accurately using HomeCare/HomeBase EMR
Build meaningful relationships while guiding patients toward improved mobility and independence

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Comfortable traveling within a 60-mile radius of Valley City

Knowledge & Skills

Strong physical therapy assessment and treatment skills
Experience with HomeCare/HomeBase EMR preferred
Excellent time management for a 5-7 patient daily caseload
Ability to work independently in home settings
Compassionate communication with patients and families
