Roles & Responsibilities

• Provide compassionate direct patient care and support with activities of daily living
• Assist with bathing, grooming, feeding, and mobility to keep patients comfortable and safe
• Take and record vital signs, reporting any changes to the nursing team promptly
• Maintain a clean, safe environment and adhere to infection control standards
• Communicate clearly and kindly with patients, families, and the care team
• Document care accurately in the patient record and follow care plans
• Support admissions, transfers, and discharges with professionalism and empathy

Qualifications

• Valid CNA certification in the state of Pennsylvania
• Current CPR/BLS certification (or willingness to obtain)
• Up-to-date immunizations and TB screening as required by the facility
• Successful background check and any required state registry verifications
• Strong communication skills and a patient-centered approach
• Ability to perform physical aspects of the job such as lifting and turning patients

Education

• High school diploma or equivalent
• State-approved CNA program completion and active CNA certification

Knowledge & Skills

• Fundamentals of patient care and activities of daily living
• Basic vital signs measurement and charting
• Infection control practices and safe handling of patient equipment
• Observation skills to notice and report changes in condition
• Teamwork and effective communication within a multidisciplinary team
• Basic electronic medical record (EMR) navigation or willingness to learn
