Roles & Responsibilities

• Implement treatment plans developed by the supervising Occupational Therapist to help residents regain independence
• Provide hands-on therapy focused on activities of daily living (ADLs), mobility, and functional tasks
• Guide residents through therapeutic exercises and meaningful functional activities
• Monitor patient progress closely and promptly report changes to the supervising OT
• Maintain accurate, timely therapy documentation to support continuity of care
• Collaborate with interdisciplinary care teams to coordinate holistic patient recovery
• Educate residents and families on safe techniques and adaptive strategies for daily living

Qualifications

• Active California Certified Occupational Therapy Assistant (COTA) license — required
• Associate Degree from an accredited COTA program — required
• Previous SNF, rehabilitation, or geriatric therapy experience — preferred
• Strong patient care focus with excellent communication skills
• Comfortable working within a multidisciplinary healthcare team
• Reliable documentation and patient monitoring abilities

Education

• Associate Degree from an accredited Certified Occupational Therapy Assistant program

Knowledge & Skills

• Experience supporting ADL training and functional rehabilitation
• Strong observation and patient monitoring skills
• Effective communication with residents, families, and care teams
• Accurate therapy documentation and charting practices
• Ability to adapt therapeutic approaches to individual needs
• Teamwork and collaboration in a skilled nursing environment
