Roles & Responsibilities

• Provide skilled physical therapy services to residents in a skilled nursing facility
• Evaluate patients, develop individualized treatment plans, and adjust interventions as needed
• Deliver evidence-based therapeutic exercises, mobility training, and functional interventions
• Document progress thoroughly and timely to meet facility and CRG policies
• Collaborate with nurses, OT, SLP, and interdisciplinary team members to optimize patient outcomes
• Meet and maintain productivity expectations (90% productivity requirement)
• Adhere to facility dress code (black scrubs) and CRG policies and procedures

Qualifications

• DPT or PT (degree) — core skillset includes PT/DPT expertise
• 1–3 years of physical therapy experience (as listed in job details)
• Experience or comfort working in a Skilled Nursing Facility (SNF) setting
• Willingness to follow CRG policies and meet productivity standards
• Ability to be on-site (in-building presence required) and maintain professional dress (black scrubs)

Education

• Doctor of Physical Therapy (DPT) or equivalent Physical Therapy degree

Knowledge & Skills

• Skilled Nursing Facility (SNF) care processes and documentation
• Patient assessment, therapeutic exercise, and mobility training
• Strong communication and teamwork in an interdisciplinary setting
• Time management and productivity tracking in a clinical environment
• Clinical decision-making focused on functional outcomes
