Roles & Responsibilities

• Provide skilled occupational therapy services to residents in a skilled nursing facility to improve independence and quality of life
• Perform thorough evaluations and create individualized treatment plans focused on ADLs, mobility, and functional goals
• Deliver evidence-based interventions and adapt plans based on resident progress
• Document assessments, treatment plans, and progress in a timely, accurate manner following facility and CRG policies
• Collaborate closely with nursing, therapy colleagues, and the interdisciplinary team to coordinate care
• Meet productivity expectations and contribute positively to the therapy team culture

Qualifications

• 1-3 years of occupational therapy experience (as listed in the job posting)
• Hold or be eligible for a valid Occupational Therapy license (state licensure required per facility policy)
• Comfortable working in a Skilled Nursing Facility (SNF) practice setting
• Ability to meet productivity requirements (90% productivity noted in job instructions)
• Willingness to follow Continuum Rehabilitation Group (CRG) policies and procedures
• Able to be present in the building to receive guaranteed hours and follow facility dress code (black scrubs)

Education

• Degree in Occupational Therapy (BS/BA/MS/OTD) or equivalent; OTR credential preferred

Knowledge & Skills

• Skilled in OT evaluations, treatment planning, and functional interventions
• Experience with ADL training, adaptive equipment recommendations, and mobility support
• Strong documentation skills and familiarity with SNF documentation standards
• Good communication and teamwork skills for collaborating with interdisciplinary staff
• Ability to prioritize caseload and manage time effectively to meet productivity expectations
