Roles & Responsibilities

• Provide skilled speech-language pathology evaluation and treatment for SNF residents
• Develop and implement individualized therapy plans to improve communication and swallowing
• Document clinical progress and maintain accurate, timely records per facility and CRG policies
• Collaborate closely with the therapy team and interdisciplinary staff to coordinate care
• Meet productivity expectations and contribute to a high-performing therapy program
• Adhere to facility dress code (black scrubs) and professional standards
• Communicate effectively with residents, families, and staff to support patient-centered care

Qualifications

• Active credentials as an SLP (state license or equivalent per facility requirements)
• 1-3 years of clinical experience (as listed in the job order)
• Experience or comfort working in a skilled nursing facility (SNF) setting
• Strong documentation skills and ability to meet productivity expectations (90%)
• Ability to be on-site (therapist must be present in the building to receive guaranteed hours)
• Willingness to follow Continuum Rehabilitation Group (CRG) policies and procedures
• Professional demeanor and adherence to facility dress code

Education

• SLP credentialing as required by state/facility (license or certification; ASHA CCC-SLP commonly expected)

Knowledge & Skills

• Skilled in adult SLP assessments and therapeutic interventions
• Knowledge of dysphagia management and safe swallowing techniques
• Proficient clinical documentation and care planning for SNF residents
• Teamwork, communication, and interdisciplinary collaboration skills
• Time management and ability to meet productivity targets
• Familiarity with SNF regulatory and documentation expectations
