Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with on-call rotation for evenings, weekends, and holidays

Hours per Week: 40

Candidate Pay Rate: $62 per hour

Roles & Responsibilities

Conduct comprehensive initial assessments of clients and families to determine home health needs
Develop and regularly update individualized care plans based on patient diagnosis and changing needs
Administer medications and treatments as prescribed while providing clear health care instructions
Coordinate care as Case Manager, ensuring seamless communication between physicians, patients, and care teams
Prepare clinical documentation and maintain open communication with physicians regarding patient status
Supervise and evaluate home health aide care to ensure quality patient outcomes
Identify discharge planning needs early and implement smooth transitions of care

Qualifications

Current Registered Nurse license in North Dakota
BLS certification required
Home Health and Hospice experience required
Valid driver's license and auto insurance
Experience working with adults, geriatrics, and pediatric populations

Knowledge & Skills

HomeCare HomeBase EMR proficiency
Strong assessment and care planning abilities
Excellent communication and patient education skills
Case management and care coordination experience
Ability to work independently in various home settings
