Roles & Responsibilities

Deliver meaningful physical therapy care to patients across four rural counties
Conduct comprehensive Oasis Assessments and Start of Care evaluations
Help elderly patients recover from falls and fractures with personalized treatment plans
Track visits and time using tablet technology for seamless documentation
Build lasting relationships with patients in their home environments

Qualifications

Active Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong assessment and clinical reasoning skills

Knowledge & Skills

Home health physical therapy expertise
Oasis documentation proficiency
Tablet-based electronic documentation
Rural healthcare delivery
Geriatric rehabilitation focus
