Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
Assess each patient's unique needs and create tailored treatment plans that promote independence and healing
Collaborate with an interdisciplinary team to ensure seamless, patient-centered care across home health and hospice settings
Educate patients and families on safe mobility techniques and exercises to support ongoing recovery
Document care accurately using HomeCare/HomeBase EMR to maintain clear communication and continuity

Qualifications

Current North Dakota Physical Therapy License
BLS certification
At least 1 year of physical therapy experience, including previous home health experience
Valid driver's license and auto insurance for travel within the service area

Knowledge & Skills

Strong clinical skills in physical therapy assessment and treatment
Comfort working independently in home settings
Familiarity with HomeCare/HomeBase EMR preferred
Excellent communication and patient education abilities
Ability to manage a caseload of 5-7 patients per day
