Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Deliver personalized physical therapy in patients' homes
Create customized treatment plans that promote independence
Collaborate with interdisciplinary care teams
Educate patients and families on recovery strategies
Document progress and adjust care plans as needed

Qualifications

Current California PT license
BLS certification
Home health experience preferred
Strong communication and time-management skills

Knowledge & Skills

Home health physical therapy techniques
Patient assessment and care planning
Mobility and rehabilitation strategies
Electronic documentation systems
