Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Conduct start of care assessments and resumption of care visits
Complete recertifications and ongoing treatment sessions
Deliver 6.5 points of care per day with a mix of regular visits and admissions
Perform safety evaluations and teaching visits in hospice situations when needed
Document patient progress and coordinate with the interdisciplinary team

Qualifications

Current North Dakota Physical Therapy License required
BLS certification required
Valid driver's license and auto insurance required
Home Health experience and OASIS training preferred
First time travelers welcome to apply

Home Health physical therapy expertise
OASIS documentation proficiency
Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently in home settings
