Roles & Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans
Assist patients in improving activities of daily living (ADLs) and functional independence
Collaborate with an interdisciplinary rehab team to support optimal patient outcomes
Educate patients and caregivers on adaptive techniques, safety, and exercises
Maintain accurate and timely documentation to support quality care

Qualifications

Active California Occupational Therapist (OT) license required
SNF or rehab experience preferred
Strong clinical judgment and effective communication skills
Ability to work independently and as part of a collaborative team

Knowledge & Skills

Occupational Therapy principles and interventions
Clinical judgment applied to patient evaluation and treatment planning
Clear communication with patients, caregivers, and rehab team members
Teamwork within an interdisciplinary skilled nursing facility setting
Accurate clinical documentation and charting
