Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and on time
• Monitor and document patient condition, reporting changes to RN/supervisor
• Support care planning and communicate effectively with interdisciplinary teams
• Assist with activities of daily living while promoting patient dignity and independence
• Maintain accurate charting and follow facility policies and procedures
• Participate in shift handoffs to ensure continuity of care

Qualifications

• Active LPN license (practical/vocational nurse) — eligible to practice in New York
• Basic Life Support (BLS) certification required; additional certifications a plus
• Comfortable with medication administration, charting, and basic clinical procedures
• Strong communication skills and a team-oriented mindset
• Previous experience in long-term care, skilled nursing, or similar settings preferred
• Reliable, professional, and committed to quality patient care

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program
• High school diploma or GED required

Knowledge & Skills

• Medication administration and documentation best practices
• Vital signs assessment and basic patient assessment
• Infection control and safety protocols
• Electronic health record (EHR) charting familiarity
• Time management and prioritization during busy shifts
• Collaborative communication with nurses, therapists, and support staff
