Roles & Responsibilities

Provide compassionate direct patient care, including ADL assistance and monitoring
Administer medications and treatments accurately and document per policy
Observe and record patient conditions, reporting any changes promptly
Collaborate with nursing staff and interdisciplinary team to support patient plans
Maintain accurate and timely documentation in the patient chart
Follow infection control and safety protocols to protect patients and staff
Support a positive, patient-centered environment through respectful communication

Qualifications

Current LPN license in Pennsylvania required
BLS/CPR certification required; additional certifications a plus
Completion of an accredited Practical Nursing program
Strong communication skills and ability to work well on a care team
Comfortable with medication administration, basic wound care, and patient assessments
Excellent attention to detail and reliable clinical judgment

Education

Graduate of an accredited Practical Nursing program
High school diploma or GED

Knowledge & Skills

Patient assessment and vital sign monitoring
Medication administration and documentation
Basic wound care and ADL assistance
Familiarity with electronic medical records and charting
Infection control practices and safety protocols
Time management and prioritization in a clinical setting
Teamwork and clear communication with clinical staff and leadership
