Roles & Responsibilities

Provide compassionate direct patient care within the scope of LPN practice at the facility
Administer medications and treatments accurately and document thoroughly
Monitor patient conditions and promptly report changes to the care team
Perform routine assessments and maintain accurate, timely patient records
Collaborate with nurses, providers, and support staff to deliver coordinated care
Support quality improvement efforts and adhere to facility policies and safety protocols

Qualifications

Active LPN license in Pennsylvania (required)
Practical Nursing diploma or equivalent
Current BLS/CPR certification or willingness to obtain upon hire
Strong communication, documentation, and teamwork skills
Previous experience in long-term care or similar setting preferred

Education

Practical Nursing diploma or certificate; active LPN license required

Knowledge & Skills

Medication administration and accurate documentation
Vital signs monitoring and basic patient assessment
Familiarity with electronic health records and charting
Infection control and safe patient handling practices
Patient-centered communication and empathy
