Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and document carefully
• Monitor and record vital signs and report any changes to the care team
• Assist with activities of daily living while promoting patient dignity
• Collaborate with nurses, therapists, and support staff to implement care plans
• Maintain infection control standards and keep a safe care environment
• Communicate clearly and empathetically with patients and their families
• Participate in orientation and ongoing training to stay current with best practices

Qualifications

• Active Licensed Practical Nurse (LPN) license — New York license recommended
• Current BLS/CPR certification or willingness to obtain it promptly
• Comfortable with medication administration, charting, and clinical assessments
• Professional, patient-centered attitude and strong teamwork skills
• Previous LPN experience in long-term care, skilled nursing, or similar settings preferred

Education

• Completion of an accredited practical nursing program or equivalent diploma

Knowledge & Skills

• Solid knowledge of vital sign assessment and documentation
• Medication administration and accurate charting skills
• Familiarity with infection control protocols and safe patient handling
• Strong communication and interpersonal skills with patients and staff
• Time management and ability to prioritize tasks in a busy clinical setting
• Basic familiarity with electronic medical records and clinical documentation
