Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents of the rehab center
• Administer medications and treatments accurately and document per facility policy
• Assess and monitor resident conditions; report changes promptly to the care team
• Support residents with activities of daily living and promote independence
• Collaborate with nurses, therapists and support staff to deliver coordinated care
• Maintain accurate, timely charting in the resident record and uphold confidentiality
• Adhere to infection control, safety protocols and facility standards at all times

Qualifications

• Current Licensed Practical Nurse (LPN) license — must be valid in New Hampshire or obtainable prior to start
• Previous experience in long-term care or rehabilitation settings preferred
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Strong communication skills and ability to work as part of an interdisciplinary team
• Reliable, professional, and compassionate approach to resident care

Education

• Graduate of an accredited practical/vocational nursing program (LPN)

Knowledge & Skills

• Medication administration and documentation best practices
• Vital signs assessment and basic clinical monitoring
• Wound care basics and assistance with treatments
• Experience with electronic medical records (EMR) or charting systems
• Time management, prioritization, and strong clinical judgement
• Clear communication with residents, families and clinical teams
