Roles & Responsibilities

• Conduct thorough speech-language evaluations to identify patient needs
• Provide daily treatments focused on functional communication and swallowing
• Plan and manage patient discharges, ensuring smooth transitions of care
• Collaborate actively with the therapy team to deliver coordinated care
• Adhere to CRG policies and facility procedures to maintain quality standards
• Maintain required productivity expectations and documentation standards
• Present professionally onsite (black scrubs per facility dress code) and be available to receive guaranteed hours

Qualifications

• Licensed or credentialed Speech-Language Pathologist (SLP) eligible to practice in New Mexico
• 1-3 years of clinical experience (as listed in the job order)
• Prior SNF/skilled nursing facility experience strongly preferred
• Ability and willingness to work on-site during daytime hours
• Comfort following facility policies and meeting productivity expectations

Knowledge & Skills

• Skilled in evaluations, daily treatments, and discharge planning
• Experience working in skilled nursing facility (SNF) therapy settings
• Strong communication and interdisciplinary collaboration skills
• Familiarity with facility policies (CRG P & P) and productivity requirements
• Professional presence and patient-centered care orientation
