Shift Type: Monday-Friday, 8:00 AM - 4:30 PM Day with weekend rotation (shared schedule; week before a scheduled weekend: Thursday off, work Saturday & Sunday, then the following Friday off)

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

• Provide skilled nursing care in patients' homes, including assisted living and adult foster homes
• Assess patients' health status, identify changes, and implement appropriate nursing interventions
• Develop, update, and carry out individualized care plans with compassion and clinical accuracy
• Educate patients and families on disease management, medications, and home safety
• Coordinate care with interdisciplinary team members and community resources
• Document patient encounters clearly and timely in the medical record
• Manage your caseload independently while escalating concerns appropriately

Qualifications

• Current BLS certification (required)
• Three or more years of nursing experience (required)
• Home Health experience preferred
• Able to work with minimal direction or supervision and manage an independent caseload
• Must have own automobile for travel to patient homes

• Strong clinical judgment and ability to prioritize patient needs
• Clear, compassionate patient and family education skills
• Reliable documentation practices and attention to detail
• Time management and route planning for efficient home visits
