Roles & Responsibilities

• Perform thorough physical therapy evaluations and develop individualized plans of care
• Deliver hands-on treatment across settings: patient rooms, hallways, outdoor areas, and the rehab gym
• Collaborate with PT/OT/SLP and the interdisciplinary team during rounds and care conferences
• Document patient care and progress accurately in Cerner and NetHealth
• Communicate proactively about equipment needs, patient progress, and discharge planning
• Provide compassionate, patient-centered rehabilitation that supports recovery and safe transitions

Qualifications

• Current NC Physical Therapy license (required)
• CPR certification (required)
• Minimum 2 years of clinical experience (position lists 2; hiring profile prefers clinicians with ~3 years in a medical setting)
• Experience in hospital and/or nursing home settings (must-have)
• Comfort floating between acute, swing, nursing home, and outpatient settings
• Familiarity with rehab equipment and outpatient/clinic workflows

Education

• Degree in Physical Therapy (DPT or equivalent preferred); state licensure required

Knowledge & Skills

• Skilled in physical therapy evaluations and plan-of-care development
• Proficient with Cerner and NetHealth documentation systems
• Strong communication and teamwork with interdisciplinary rehab staff
• Experience with discharge planning and patient-centered care coordination
• Ability to safely use and instruct patients with standard rehab gym equipment
