Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day); plus minimum 2 weekend days per month (typically one Saturday and one Sunday on separate weekends). May be floated to nearby Swedish campuses (Issaquah, First Hill, Ballard).

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

• Conduct thorough biopsychosocial assessments to identify patient and caregiver needs
• Develop and implement individualized plans of care that support safe discharge
• Coordinate care across disciplines and with community providers to ensure continuity
• Provide crisis intervention and high-risk patient safety planning when needed
• Facilitate discharge planning, referrals to rehab and long-term care, and DME orders
• Document care and coordination activities accurately in Epic according to plan of care
• Educate patients and families about advance directives, benefits, and community resources
• Participate in care conferences and collaborate with PT, RT, pharmacy, and other teams

Qualifications

• Required certifications: BLS and LSWAIC
• Epic charting experience required
• Minimum 1 year experience in inpatient/acute care case management
• Prior hospital experience preferred (experience working with neuro and cardiac patients a plus)
• Comfort with floating to nearby campuses and meeting weekend coverage needs
• Strong assessment, crisis intervention, documentation, and care coordination skills
• LCSW licensure or MSW degree is preferred

• MSW preferred

Knowledge & Skills

• Inpatient case management and discharge planning
• Biopsychosocial assessment and care planning
• Crisis intervention and suicide prevention strategies
• Knowledge of community resources: Medicaid, Medicare, housing assistance, food assistance programs
• Understanding of guardianship, power of attorney, and protective services
• Familiarity with ICU and medical-surgical patient needs
• Experience coordinating home care, equipment, and prior authorization processes
• Strong Epic documentation and electronic charting skills
