Roles & Responsibilities

• Provide compassionate, hands-on nursing care to assigned patients
• Administer medications and treatments safely and document per policy
• Monitor and record vital signs and recognize changes in patient condition
• Collaborate with the interdisciplinary team to support patient-centered care
• Maintain accurate, timely charting and assist with care planning
• Support patient and family education, promoting comfort and recovery

Qualifications

• Valid Licensed Practical Nurse (LPN) license in good standing
• Current Basic Life Support (BLS) certification or willingness to obtain
• Comfortable with medication administration, basic wound care, and patient assessments
• Professional, reliable, and able to pass facility background checks and health requirements
• Prior experience in long-term care or med-surg settings is a plus

Education

• Graduate of an accredited practical nursing program (LPN/LVN)

Knowledge & Skills

• Strong fundamentals in medication administration and documentation
• Skilled in patient assessment and monitoring vital signs
• Comfortable using electronic health records and following facility protocols
• Excellent communication and teamwork with nurses, providers, and families
• Ability to prioritize tasks in a fast-paced clinical environment
