Roles & Responsibilities

• Assess and evaluate patients' speech, language, cognitive-communication, and swallowing needs in a skilled nursing setting
• Develop individualized treatment plans and implement evidence-based therapy interventions
• Document clinical progress thoroughly and timely to meet facility and CRG policies
• Collaborate with the therapy team, nursing staff, and families to support patient-centered care
• Meet productivity expectations and contribute to department goals (90% productivity requirement)
• Adhere to facility dress code and presence requirements to ensure guaranteed hours

Qualifications

• 1-3 years of SLP experience (as listed in the job order)
• Experience working in a skilled nursing facility or similar therapy setting preferred
• Ability and willingness to be present on-site to receive guaranteed hours and meet productivity expectations
• Comfort following Continuum Rehabilitation Group (CRG) policies and procedures
• Professionalism in dress and conduct (black scrubs required by facility)

Knowledge & Skills

• Skilled in Speech-Language Pathology evaluation and treatment in SNF settings
• Strong documentation and clinical reporting skills
• Ability to work effectively within a multidisciplinary therapy team
• Familiarity with productivity expectations and meeting department metrics
• Good interpersonal skills for patient, family, and staff communication
