Roles & Responsibilities

• Assess residents’ mobility and functional needs to create individualized therapy plans
• Implement therapeutic exercise, gait training, transfers and functional activities to improve patient outcomes
• Collaborate with the interdisciplinary team to coordinate care and support resident goals
• Document evaluations, progress notes and discharge plans accurately and timely per CRG policies
• Meet productivity expectations while maintaining high-quality, person-centered care
• Communicate with family members and caregivers to support continuity of care and education

Qualifications

• PT or DPT credential (PT/DPT listed as the job skills)
• 1-3 years of experience (as listed in the posting)
• Experience or comfort working in a Skilled Nursing Facility (SNF) practice setting
• Willingness to follow CRG policies & procedures and meet the facility’s productivity requirement (90%)
• Able to be on-site to receive guaranteed hours and adhere to dress code (black scrubs)
• Able to complete interview with Regional Director of Therapy as part of the hiring process

Knowledge & Skills

• Skilled in physical therapy evaluations, treatment planning and functional mobility training
• Familiarity with SNF documentation, regulatory expectations and productivity tracking
• Strong communication and teamwork for working with nursing and rehab staff
• Ability to prioritize caseload, manage time effectively, and deliver efficient patient care
• Professionalism and adherence to company policies and facility procedures
