Roles & Responsibilities

• Evaluate residents’ speech, language, cognition, and swallowing needs using best practices
• Develop and implement individualized treatment plans that improve communication and safety
• Document clinical progress and maintain accurate records in line with facility policies
• Collaborate with nurses, therapists, and interdisciplinary team members to coordinate care
• Educate residents and families on strategies to support communication and swallowing at home
• Adhere to Continuum Rehabilitation Group and facility policies, including dress code and onsite presence

Qualifications

• Primary skill: SLP (Speech-Language Pathology) experience
• 1-3 years of clinical experience (as specified in the order)
• Ability to be present in the building to receive guaranteed hours and meet productivity expectations
• Willingness to follow CRG policies and facility procedures, including dress code of black scrubs
• Comfort working in a skilled nursing facility (SNF) practice setting

Knowledge & Skills

• Familiarity with SNF therapy practices and resident-centered care
• Skilled in assessment and treatment planning for speech, language, cognition, and swallowing
• Strong documentation and time management skills to meet productivity requirements
• Collaborative skills for working within an interdisciplinary rehab team
• Professionalism in following facility policies and maintaining clinical standards
