Roles & Responsibilities

• Prepare and accurately dispense medication orders to support safe, effective patient care
• Compound and prepare IV admixtures, chemotherapy, and parenteral nutrition with precision
• Maintain medication stock in patient care areas and manage controlled substance documentation
• Perform pharmacokinetic dosing and drug therapy monitoring per hospital protocols
• Review culture and sensitivity data and support antibiotic stewardship initiatives
• Provide clinical consultation and drug information to clinicians, patients, and families
• Document clinical activities and interventions clearly and participate in quality improvement
• Support formulary management by suggesting therapeutic substitutions and minimizing non-formulary use
• Participate as a member of the Code Response Team, responding professionally in high-stakes situations
• Assist with orientation, education, and training for pharmacy and clinical staff

Qualifications

• Active CO Pharmacist License in good standing (required)
• Epic experience required; Meditech experience is a plus
• Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree
• BLS card (American Heart Association) required
• ACLS card (American Heart Association) required
• Flu vaccine required (no exemptions accepted during Oct 1 - Apr 1)
• Experience with medication dispensing, compounding, IV admixture, and pharmacokinetic dosing
• Experience participating in code response or emergency response teams is a plus

Education

• BS Pharmacy from an ACPE-accredited School of Pharmacy

Knowledge & Skills

• Epic electronic medical record workflows and medication profile management
• Medication dispensing best practices and IV admixture compounding
• Pharmacokinetic dosing and therapeutic drug monitoring
• Formulary management and therapeutic substitution strategies
• Antibiotic stewardship and interpretation of C&S data
• Chemotherapy compounding and parenteral nutrition preparation
• Clinical consultation skills and clear documentation of interventions
• Familiarity with Meditech is a plus during EMR conversion
